Recovery Resource Council is looking for a conscientious, detail-oriented, and compassionate individual who is capable of successfully managing multiple tasks and responsibilities to assist Veterans who are currently homeless or at imminent risk of homelessness by 
providing rapid rehousing and homeless prevention services.

Skills / Requirements:

To perform this job successfully, an individual must have a Bachelor’s degree in Social Work or related field and/or 2 years of experience providing case management in the human service field. Experience working with people experiencing homelessness and/or veterans is preferred. Must have reliable transportation, a current Texas driver’s license and evidence of auto liability insurance.

Responsibilities:
The SSVF Case Manager is responsible for providing case management services to veterans and their families who are either at imminent risk of homelessness or are currently experiencing homelessness. The Case Manager conducts intakes, housing navigation, develops Housing Stability Plans, provides case management to assist the veteran with housing stability goals, and follows guidance and regulations of SSVF to provide financial assistance. The Case Manager also works with the Health Care Navigator and the Employment Specialist to ensure participation with additional services to enhance stability. The Case Manager is responsible for creating and maintaining positive working relationships with properties in the area. The Case Manager will also participate and represent the program in community team meetings, case conferences, and other staff meetings. The Case Manager is responsible for timely and accurate documentation of services and activities in HMIS and the agency’s client records.

This is a hybrid position, requiring both remote and in-person work, involving sitting and standing for various lengths of time and frequently driving for long distances.
